New York State Commission on Quality of Care Surrogate Decision Making Committee
Program
This program provides specially trained volunteers who make medical treatment decisions
on behalf of people who do not have the capacity to make their own and who have
no legally authorized and willing surrogate to make medical decisions on their behalf.
Each panel of volunteers includes a medical professional, a family member, an attorney,
and an individual with interest and the expertise in the care and treatment of individuals
with mental disabilities. Services are available in Erie, Niagara, Genesee, Orleans,
Wyoming, Allegany, Cattaraugus and Chautauqua counties. There is no cost to participants
in this program.
